PLEASE READ ALL INFORMATION BELOW BEFORE SCHEDULING AN APPOINTMENT.

Please INDICATE WHAT SERVICE YOU ARE SCHEDULING by adding it in parentheses after your name.

EACH applicant requesting a service must book their own slot/appointment.

Please be on time for your scheduled appointment.

We cannot accept incomplete, improperly filled out, or handwritten applications.

For NOTARIAL applicants:
• Please bring the form you would like to sign and have notarized and a valid government-issued photo ID.
If your document requires witnesses, your preferred witnesses should accompany you to your appointment and to bring their valid government-issued SIGNED photo IDs.

For PASSPORT applicants:
• Please visit our passport navigator (https://tinyurl.com/bdzxpdr3) for guidance on required passport application forms and the required supporting documents.
• Please note: you MUST fill out required forms for your service ahead of your appointment, and they must be typed and printed out. We cannot accept handwritten applications.
• If your last passport was lost or stolen, please ensure you fill out a DS-64 documenting this loss or theft: https://eforms.state.gov/Forms/ds64.pdf
• The wait for regular passport processing is currently 3-4 weeks, so please plan ahead.

You must pay for your DS-11 or DS-82 application ahead of time!

For official and diplomatic passport applicants: In addition to the requirements explained in the passport navigator, please also bring:
• State Department: An electronic request for passport services issued by your HR Technician in Washington. Visit https://gtmprod.servicenowservices.com/next, select "service catalog" then "assignments and travel," then "Request for Passport Services Form."
(DS-1640).
• Department of Defense: An electronic request for passport services (DD-1056) issued by usarmy.belvoir.hqda-oaa-det.mbx.sia-eirl-agent-notification@army.mil AND your orders.
• For all other agencies: Please email Amman-ACS@state.gov to confirm the authorizing document required for your official or diplomatic passport.
• The current minimum processing time for diplomatic and official passports is 6-8 weeks. Please plan ahead, as expedite requests rarely make a significant difference in processing time.
